Narrative

With four full-time adult cardiac surgeons and four general thoracic surgeons at Beth Israel Deaconess Medical Center, the varieties of operations include:

  • VATS lobectomy and minimally invasive esophagectomy (MIE)
  • Tracheal resection and tracheobronchoplasty
  • Minimally invasive and “off-bypass” coronary revascularizations
  • Aortic homografts
  • Aortic root operations
  • Exposure to advanced bronchoscopy (e.g. EBUS, stents, rigid, cryotherapy)
  • Other major aortic procedures

Teaching by outstanding Harvard Medical School faculty enables the resident to become proficient in the operative and perioperative care of patients. Progress toward technical proficiency is evaluated at multiple points during the residency. Multiple educational sessions occur throughout the week on each service, including preoperative case conference, core curriculum conference, professor rounds, clinical case conference and cardiac catheterization conference. In addition, monthly journal clubs and multidisciplinary airway conferences are held. Participation in the Harvard Combined Cardiothoracic Conference is mandatory.

Our two-year program consists of:

  • Six months of general thoracic surgery at BIDMC
  • Nine months of adult cardiac surgery at BIDMC
  • Two months of pediatric cardiac surgery at Boston Children’s Hospital
  • Seven months of adult cardiothoracic surgery at BIDMC — a chief resident experience on the adult cardiac surgery or general thoracic surgery service, a flexible designation depending on the trainee’s career goals

General Thoracic Surgery

The first six-month block of the first year is composed of General Thoracic Surgery.

  • Participation in the preoperative, operative and postoperative care of all patients, with the trainee serving as chief resident of the service
  • Gradually increased operative autonomy, including complex minimally invasive pulmonary and esophageal operations, complex airway surgery and surgical treatment of airway disease
  • Participation in the weekly Multidisciplinary Thoracic Clinic, followed by a conference at which thoracic surgeons, medical oncologists, radiation oncologists, pulmonologists and thoracic radiologists discuss the patients seen earlier in the day
  • Close collaboration with Interventional Pulmonology, including a two-week rotation learning EBUS, rigid bronchoscopy with advanced endoscopic intervention, pleural ultrasound, and minimally invasive pleural interventions

Adult Cardiac Surgery

The second six-month block of the first year and beginning three-months of the second year is composed of Adult Cardiac Surgery.

Exposure to:

  • Coronary revascularization
  • Valve repair and replacement
  • Aortic surgery and repair of congenital defects seen in adult patients
  • Repair of atrial septic defects

Participation in preoperative, operative and postoperative care

Residents are involved in the operative care at an early stage, but only at a time when the attending surgeon feels comfortable allowing the resident to perform an active role

  • Training in transesophageal echocardiography

Boston Children’s Hospital

During the second year, in a two-month block, the resident is at Boston Children’s Hospital, where the resident is exposed to congenital cardiac surgery in infants, adolescents and young adult surgery. Pediatric cardiac surgery is an integral component of the specialty.

Adult Cardiothoracic Surgery

During the second half of year two, in a seven-month block, the resident spends time on the Adult Cardiac Surgery or General Thoracic Surgery service. This will be dependent on the individual resident’s eventual career focus and exposure to that point. The resident functions as the chief resident on either service.

The Cleveland Clinic offers a 2-year Thoracic Fellowship, General Thoracic Track. This addition of this program allows a training pathway for those trainees who have completed a general surgery residency with the desire to train in Thoracic Surgery.  Trainees accepted into this program will join a General Thoracic Surgery section at the Cleveland Clinic that performs in excess of 2000 surgeries each year. They will work on the service with six teaching faculty.  Fellows will assist the faculty  in a spectrum of surgeries ranging from  minimally invasive (including robotic) lung, foregut, and mediastinal procedures to extensive oncologic resections utilizing cardiopulmonary bypass.  They will train with a group of surgeons that perform over 100 index esophageal surgery operations yearly treating benign and malignant etiologies.  These operations will familiarize them with open, laparoscopic, robotic, and endoscopic approaches.  Trainees will be exposed to a unique volume of tracheal operations with the option of taking an elective in interventional bronchoscopy.  Pulmonary transplant and Extracorporeal support is an important component of the program and trainees will have the opportunity to complete a dedicated elective in this discipline should they so desire.   They will find themselves recipients of exceptional mentorship provided by general thoracic faculty who are highly academic, well-published, and active members of numerous national societies and organizations.

There are few thoracic surgery groups across the United States that can offer the same spectrum of exposure to their trainees while guiding their development into general thoracic surgeon leaders of the future.

The Training Program in Thoracic Surgery at Duke University Medical Center is designed to develop academic cardiothoracic surgeons. At the completion of the program, residents will demonstrate proficiency in all clinical aspects of cardiothoracic surgery and will have extensive experience and training in clinical research.

The training program, a 3-year program with 3 residents at each training level, provides for progressive responsibility and a balanced exposure to adult cardiac surgery, congenital cardiac surgery, and general thoracic surgery. The philosophy of the training program is predicated on the mentorship system, where each resident rotates on a service with 2-3 faculty members for 4 months, providing consistent, sustained relationships. Unique to our program, there are 2 VA hospital rotations, one in Durham and one in Asheville, where autonomy is optimized.

The Thoracic Surgery Service at Memorial Sloan Kettering Cancer Center (MSKCC) offers an ACGME accredited two-year thoracic-track cardiothoracic residency program. This program was originally established by Dr. Robert Ginsberg in 1990 with the mission of training future leaders in thoracic surgery, and was one of the first programs in the country to provide a focus on general thoracic surgery. Montefiore-Einstein Medical center offers 2 traditional Cardiothoracic Fellowship positions with a comprehensive and broad-based experience in adult cardiac surgery, general thoracic surgery, and congenital heart surgery. For the 2022 interview season, interviews will be held virtually via Zoom. Prior to the day of the interviews, residents will be emailed their individual agendas for the day.  An informal Zoom cocktail hour will be held the night before the interview. Each candidate is virtually interviewed by at least 3 NYPH/WMC attendings.  In between interviews, candidates will have the opportunity to ask questions about the program from current fellows located in a Zoom breakout room.  At the same time, we will have shared screens available with a slideshow presenting additional information about program.

The program offers 3 ACGME approved positions: 2 cardiac fellowship positions and 1 thoracic fellowship position. Our thoracic program is under the direction of Dr. Nasser K. Altorki at NewYork-Presbyterian|Weill Cornell Medicine.  This 15/9 thoracic track program is based at WCMC and includes 15 months of thoracic exposure, with rotations at Memorial Sloan Kettering.  The remaining 9 months are cardiac training at WCMC.  Our cardiac track will continue to include 3 months of thoracic training at Cornell, 3 months at the Memorial Sloan Kettering Cancer Center (6 months total thoracic training) and 18 months of cardiac training at Cornell (including network locations in Brooklyn, Queens and CHONY). There will be a separate NRMP code for the 2 tracks, and track designation by the applicant is required.

Narrative
The Penn State Health Thoracic Surgery Residency is a comprehensive two-year ACGME-accredited fellowship program. The ten cardiothoracic surgery faculty run two busy services: one that covers the gamut of adult cardiac and thoracic surgery procedures and one the offers broad exposure to congenital cardiac surgery.  The first year focuses on pediatric cardiothoracic surgery then general thoracic surgery, including open, minimally invasive (VATS and robotic), and endoscopic interventions for malignant and benign lung, esophageal, mediastinal, tracheal, chest wall, pleural, and diaphragmatic disorders. The second year concentrates on adult cardiac surgery including heart failure surgery, valvular surgery, aortic surgery, CABG, endovascular interventions, and cardiac transplantation. Trainees can expect to perform at least 350 operations as primary surgeon. They can also participate in an extensive ventricular assist device and artificial heart program if so inclined. In the Penn State Heart and Vascular Institute, the opportunity to experience other cutting edge treatment modalities is encouraged.

The Texas Heart Institute/Baylor College of Medicine Thoracic Surgery Residency Program is a three-year program that accepts four residents annually.

The program’s core values are integrity, respect for the individual, continuous commitment to excellence, dedication to discovery/innovation, and fostering leadership through education and teaching.  These values are expressed through a program providing comprehensive training in adult and pediatric thoracic and cardiac surgery in an environment that stresses clinical excellence, clinical and basic research, and teaching as long-term goals for graduates.  To be eligible to apply, candidates must have successfully completed five years of an accredited General Surgery Residency Program and be eligible for examination by the American Board of Surgery.

The Texas Heart Institute/Baylor College of Medicine Thoracic Surgery Residency Program provides thorough training in all aspects of pre-operative and post-operative care to patients with pathological conditions within the chest.  These include coronary artery disease, valvular heart disease, diseases of the aorta and great vessels, heart failure, lung failure, lung cancer, emphysema, disorders of the trachea and airways, esophageal disease (benign and malignant), and disorders of the mediastinum.  A large and growing program in congenital heart surgery at Texas Children’s Hospital also provides thoracic residents significant experience treating congenital heart disease. Other clinical rotations include renowned institutions: University of Texas MD Anderson Cancer Center/University of Texas Medical School at Houston; Harris County Hospital District/Ben Taub General Hospital; Michael E. DeBakey Veterans Administration Hospital; and Texas Heart Institute at St. Luke’s Episcopal Hospital.

The program emphasizes the central role of the residents in the management of thoracic surgical patients. It strives to impart core values as lifelong goals for the residents and seeks to help them become educators for life.  In recognizing the relatively senior status and long clinical experience of the thoracic residents – and the highly competitive nature of the selection process for this program – residents are viewed as colleagues as well as students, and it is considered a privilege to work with them.  Residents obtain a large and diverse clinical experience of decision-making and hands-on operation experience.

The University of Arizona’s Banner University Medical Center (BUMC-T) is a 365-bed tertiary medical center with 2 dedicated cardiothoracic surgery operating rooms, an adult cardiothoracic intensive care unit, and a pediatric intensive care unit.

The program matches one resident per year for a 2-year residency. The two residents alternate between the cardiac and thoracic surgery services.  There are no other surgical residents or trainees on either service to compete for cases. Six midlevel practitioners (ANPs or PAs) support the two services.

The Banner University Medical Center has active heart and lung transplant programs. The mechanical circulatory support program includes short- and long-term support devices (VAD and Total Artificial Heart). The hospital also has a Perfusion Sciences program (graduation 4 Perfusionists per year), an Artificial Heart Department, and an ECMO program with ECMO transport capability.  The CT Surgery residents are involved with all transplant and assist device cases.

University of Arizona Cancer Center is one of 41 comprehensive cancer centers in the nation, and the only NCI designated comprehensive cancer center in the state of Arizona. The thoracic surgery program performs all types of pulmonary and mediastinal resections, esophageal resections, operations for benign esophageal disease, tracheal resection and reconstruction for both benign and malignant disease. All of the most updated technologies in general thoracic surgery, including thoracoscopic (VATS) and robotic surgery, are performed at BUMC. The residents are involved in management and treatment of patients in a multi-disciplinary approach, working closely with clinicians in different disciplines, nurses, and scientists. The residents are given opportunity to be involved in both basic science and/or clinical research and are mentored by faculty members.

The CT Surgery resident currently rotates to our sister institution, Banner Cardon Children’s Hospital in Phoenix, for 2-3 months of congenital heart surgery experience.  This may change, as the congenital heart surgery is re-developed in Tucson at BUMC, potentially allowing the resident to stay in Tucson.

Each weekday begins with a didactic conference: Monday, Cardiology/CT Surgery catheterization conference; Tuesday, Thoracic Surgery Directors Association (TSDA) core curriculum, (and later multidisciplinary thoracic tumor board); Wednesday, CT Surgery Grand Rounds alternating with Morbidity/Mortality conference; Thursday, CT Surgery/Cardiology Heart Team difficult joint case conference; Friday, heart transplant selection and management conference. Brief ICU rounds follow conference before heading to the operating room each day.

The CT residents provides continuity of care by seeing most inpatient consults, and by seeing patients jointly with the attending in clinic pre- and post-op.

The CT residents have office space within the Division, which encourages frequent interaction.  Our approach is to invest time and effort in the residents to make each a competent, thoughtful, and independently operation CT surgeon upon graduating, whether in an academic or private setting.

The formal visits to our program consist of a casual meet and greet dinner with the Program Director, Associate Program Director, current Fellows, Program Coordinator and CT faculty the night prior to the interview day. This provides the applicants the chance to meet and understand the areas of interest for each of the faculty.

The interview day will start with a program overview and presentation by the Program Director at 7:30 a.m. Interviews will start promptly at 8:00 a.m. and conclude at 12:00 p.m. If time permits, the Chief fellow will give the applicants a tour of the facilities.

The Cardiothoracic Surgery residency program at the University of California, San Diego is designed to train surgeons to become leaders in the discipline of cardiothoracic surgery.  Our program strives to instill the knowledge, skills, clinical judgment, and attitudes necessary for our graduates to excel as cardiothoracic surgeons.  To accomplish these objectives, we combine a fundamental education in adult and congenital cardiac and thoracic surgery with a rich exposure to clinical material.

Our residency program is a two-year clinical program, divided into three busy services: 1) the UCSD Sulpizio Cardiovascular Center service, 2) the San Diego VA Medical Center service, and 3) the San Diego-Rady Children’s Hospital service.  Each of our services has particular strengths and breadth of operative experience.  At the conclusion of training, residents will be eligible for American Board of Thoracic Surgery Certification.

The UCSD Cardiothoracic Surgery Program accepts one thoracic surgery applicant per year, using the ERAS on-line application system.  Positions are filled via the Thoracic matching program of the National Resident Matching Program (NRMP).  All resident applicants must register with the NRMP in addition to submitting an application to our program.  Our program accepts applications from resident physicians who will have completed an ACGME accredited general surgery residency program and begin fellowship in July 2025.

The Vanderbilt University residency education program in Thoracic Surgery was established in 1957 by Rollin A. Daniel, Jr., M.D., and the program was accredited by the ACGME in 1972 under the leadership of Harvey Bender, M.D. Both the Departments of Cardiac and Thoracic Surgery maintain a focused dedication to teaching, scholarship and discovery, as well as service to patients and the community.

Our philosophy is to:

  • Provide a robust, appropriately supervised setting in which advanced, highly skilled education in adult and pediatric cardiac surgery and thoracic surgery can occur.
  • Seek opportunities for the resident to rapidly acquire an in-depth knowledge and advanced skill in cardiac and thoracic surgical techniques and to refine both surgical judgment and technical expertise.
  • Develop the resident academically and professionally. Competencies to be acquired include those of medical knowledge, patient care, practice-based learning and improvement, communication and interpersonal skills, professionalism, and systems-based practice.
  • Provide a robust clinical environment which allows graduated clinical and professional advancement accomplished by direct expert clinical, academic and professional instruction promoting self-directed education, and fostering the enhancement of the individual resident’s clinical professional, social and academic pursuits.

Our program is 3 years in length, which includes training in thoracic, cardiac and congenital cardiac surgery. Two new residents are appointed each year. This provides a balanced training curriculum with the full spectrum of cardiac and thoracic surgical problems seen during the three-year program of study.

All applicants are required to apply through the Electronic Residency Application Service (ERAS), participate in our interview process, as well as participate in the National Resident Matching Program. All documents must be received by the program via ERAS.

The Thoracic Surgery Resident Education program will be seeking 1 resident for this match cycle. We will offer a virtual happy hour the Sunday before the interviews with our current 2021-2022 residents. The potential applicants will be provided with an invitation for interview and will complete the interview on Zoom. The day of interviews the potential applicants will rotate in different meeting rooms with our faculty members.

Annually, the Cardiothoracic Fellowship Program offers one accredited position for applicants who meet the following criteria.
• Applicants must have completed a General Surgery residency and either be certified or eligible for certification in Surgery by the American Board of Surgery. • International Medical Graduates must hold a valid Educational Commission for Foreign Medical Graduates (ECFMG) certificate and obtain a J-1 visa.
• Applicants must participate in the National Resident Matching Program (NRMP) and apply through the Electronic Residency Application Service (ERAS).
• Applications must be completed by December 31 of the preceding year. Applications are available and accepted through the Electronic Residency Application Service (ERAS).

The Yale University School of Medicine offers a six-year integrated cardiothoracic surgical training program. This new program offers a streamlined curriculum that aims to prepare its trainees to develop a successful career in academic cardiothoracic surgery but derives its foundation from a rich history of traditional cardiothoracic surgical training.

The Yale integrated program curriculum is flexible, offering specialty pathways in either cardiac or general thoracic surgery leading to certification by the American Board of Thoracic Surgery (ABTS).

The first three years of the curriculum are designed to provide broad training in both surgical and medical specialties, gradually increasing both surgical responsibility and patient complexity. Environments include outpatient clinic areas, the operating room and inpatient floors (including intensive care units for postoperative cardiothoracic patients). During this period, rotation length is designed to coincide with the general surgery training program, and is assigned to maximize operative experience in general, vascular and cardiothoracic surgery. The cardiac pathway includes emphasis on vascular and endovascular surgery, as well as cardiac transplant and assist devices, minimally invasive and robotic surgery, cardiac catheterization, echocardiography, and cardiac electrophysiology. Similarly, the general thoracic pathway includes emphasis on surgical oncology, interventional radiology, and foregut/esophageal physiology. After a resident completes PGY 3, if they are interested in a substantive research experience, individual research opportunities are available and encouraged.

The last three years of the program (PGY 4, PGY 5 and PGY6) consist of intensive operative experiences in cardiac, congenital and general thoracic surgery. These experiences are individualized according to the residents’ clinical interests, yet comfortably achieving ABTS case requirements for certification.  During this period, residents will also assume administrative responsibilities to derive experience in managing their respective clinical services and organizing educational curricula.

Yale will offer two integrated cardiothoracic surgical resident positions each year.

The Yale six-year integrated program participates only in the National Residency Match Program (NRMP), therefore eligible applicants considering this program must apply via the Electronic Residency Application Service (ERAS).
